Northern Counties Health Care, Inc. (NCHC) Health Centers are family based health centers created to make quality and compassionate primary health care and health education readily available to area residents. Our Health Centers offer complete, coordinated, and individualized health care to people of all ages; from babies to great-grandparents and fill the role of the old time family doctor.
We have designed our Health Centers to include the most modern approaches to health care. We provide a one-stop location for many family health needs, including primary medical care, behavioral health, laboratory tests, and a variety of health maintenance programs aimed at the prevention of illness. In Hardwick, Orleans, and Island Pond, we also offer a comprehensive array of dental services.
The staff at each NCHC Health Center includes physicians who are board certified specialists in Family Practice, Internal Medicine and/or Pediatrics. Care is also provided by practitioners (physician assistants and nurse practitioners) who work in partnership with the supervising physicians at the Health Centers.
Each Health Center/Medical Home provides:
