- Prescription Costs
- Transportation Needs
- Limited Dental and Eyeglass Vouchers
- Connecting with Community Resources
- Support and Coaching
- Help navigate mental health and/or substance use treatment and challenges
- Coordinate hospital or ER care transitions
We have Certified Financial Coaches with Green Mountain United Way to help patients learn to set and achieve their financial goals through budgeting, debt management, loan applications, and other tools and support.*

We have Certified Application Assistants trained by the Department of Vermont Health Access to help Vermonters enroll and maintain health coverage through Vermont Health Connect or Green Mountain Care.
