We offer integrated treatment for substance use disorders (SUD) within our primary care and behavioral health services. This means you can receive medical care, counseling, and recovery support—all in one place, from a team that knows you.
Our Approach
We treat substance use disorders as a treatable medical condition, using evidence-based care tailored to your goals.
Our integrated care model includes:
- Medication for Opioid Use Disorder (MOUD), such as buprenorphine and Vivitrol
- Treatment and support for alcohol and other substance use disorders
- Same-day or rapid access to care when possible
- Coordination with your primary care provider

What to Expect
What the Intake Process Looks Like
Step 1: Meet With the MOUD Team You’ll start with an intake appointment where we learn about your health, goals, and treatment needs.
Step 2: First Visit with a Primary Care Provider After your intake, you’ll have a short visit with a primary care provider. We’ll talk about immediate concerns and begin your care plan. We strive to schedule this appointment before you run out of your medication so there is no gap in treatment.
Step 3: Release of Information (ROI) You may be asked to sign a permission form (ROI) so we can safely and legally coordinate with other providers involved in your care.
Step 4: Full New-Patient Visit
This is a longer “get-to-know-you” visit. These visits are usually booked a few months after your first visit with your provider. At this visit, we’ll go over your full health history, routine care, and any non-urgent needs. Follow-Up Visits You and your care team will work together to build a treatment plan, and your follow-up schedule will be based on that plan. This may mean coming in as often as once a week at first, but visits usually spread out over time as we get to know you and understand your treatment needs. Our goal is to create a plan that feels supportive, realistic, and manageable for you.
