Our Intake Department will discuss your options, answer questions, arrange services, and clarify insurance coverage. We offer coordinated interdisciplinary services to address any home care needs, and work in partnership with referring physicians and other providers to deliver coordinated care from the simplest to the most complex cases.
If calling after hours or on a weekend or holiday, please leave your referral request and our Intake Department staff will call you (or someone whom you designate) on the next business day to confirm acceptance of the referral. Please speak slowly and include the following information:
- Your name
- Telephone number where we can call you back
- The name of the patient you are referring (please spell the name) and their date of birth
- And, if you are calling from a physician’s office or a hospital, your department or floor
- You may fax additional information to the Intake office at (802) 748-4628
