The goal of Caledonia Home Health Care & Hospice’s long-term care program is to help participants maintain independence in the setting they prefer, to enhance the quality of life for both the client and the caregiver, and to acknowledge and value the contribution of each family member.
Our staff provides assistance with activities of daily living such as housekeeping, meal preparation, companionship and respite services to enable both client and caregiver to remain an integrated, active, and vital part of their community.
To be eligible for these services, you must be eligible for Medicaid. If you are not eligible for Medicaid, you may pay privately for services. Please visit our Private Duty section to learn more.

- The Adult High-Tech Program is designed to allow technology-dependent individuals, both children and adults, to live in their own homes. With case management and nursing shift care, families may be able to manage some very complex care needs. This program also provides support and relief for families of adults who need technical support and who require skilled nursing to be at home.
- Choices for Care is a long-term care program, funded by Medicaid, to pay for care and support for older Vermonters and people with physical disabilities that offers an alternative to nursing home care. Under this service, clients can receive case management services, homemaking, personal care assistance, meals, companionship and respite care. What you can expect from your CHHC Case Manager in the Choices for Care Program:
- Help to understand the eligibility criteria and options available to your through the Choices for Care program.
- Assistance to you and your family in navigating the application process for Long-Term Care Medicaid to help finance your participation in Choices for Care.
- Assistance obtaining durable medical equipment or home modifications to keep you safe and comfortable in your home.
- Advocacy and coordination of services with other agencies and providers.
- A monthly visit in your home to monitor services and assess your strengths, needs and eligibility for additional benefits and services.
- Our Homemaker Assistance program helps with housekeeping, laundry, meal preparation, shopping and errands.
- Support And Services at Home (SASH) brings a caring partnership together to support aging at home. The partnership connects the health and long-term care systems to nonprofit affordable housing providers statewide. Together these systems can facilitate streamlined access to the services necessary to remain safely at home.
- SASH is part of Vermont’s health care reform model, Blueprint for Health. The Blueprint for Health is organized around a medical home model through which coordinated care is provided by an interdisciplinary community health team (CHT) that supports the patient’s primary care physician (PCP). The SASH teams will extend the work of the CHTs and PCPs by providing targeted support and services to SASH participants in their homes.
