We accept most private/commercial insurances, Medicaid, and Medicare. Our federally funded health care program allows us to offer a wide range of primary and dental services to patients through a sliding-fee scale program.

Our Community Health Workers are trained by the Department of Vermont Health Access to help Vermonters enroll and maintain health coverage through Vermont Health Connect or Green Mountain Care.
For Northern Counties Healthcare Patients – We are Certified Financial Coaches with Green Mountain United Way to help patients learn to set and achieve their financial goals through budgeting, debt management, loan applications, and other tools and support.

Please bring your insurance card, and be prepared to pay your co-pay, if applicable, at the time of each visit upon check-in.
For Medicare patients, after Medicare and secondary payers have applied payment to your claim, you will be billed for the remainder of the balance. The sliding-fee scale will then be applied to this balance.
Many of our patients are also eligible for Vermont’s health and dental care programs. These State and Federally funded programs offer affordable coverage for qualified Vermont residents and their families. Health Insurance Exchanges
The Affordable Care Act (ACA), is a United States federal statute signed into law by President Barack Obama on March 23, 2010. The ACA aims to increase the quality, affordability, and rate of health insurance coverage for Americans, and reduce the costs of health care for individuals and the government. Vermont Health Connect is Vermont’s Health Insurance Marketplace.
Sliding-Fee Scale Discount Program
Northern Counties offers reduced fees to eligible primary care and dental patients who meet income guidelines as set forth each year by the Federal Government. Eligibility is based on income and family size. The program is offered to make health care more affordable for those who need assistance. Patients interested in applying for the sliding-fee scale must fill out an application and provide proof of income as described below:
Acceptable forms of Income Verification:
- Most recent Federal tax return
- IRS form W-2 or 10992 recent consecutive paystubs
- Social Security, disability or pension benefit statements
- Documentation of other governmental assistance
- Verification of Student status and FAFSA form
- Unemployment Benefits
- Patients with no income will need to speak with a patient account representative
Depending on which sliding scale level qualified for, fees are reduced to a lower specific percentage of charges or to a fixed charge per service.
Enrollment or re-enrollment is required each year.

Self-Pay Patients
For uninsured medical and dental patients who are not eligible for our sliding-fee scale, we offer a 10% Time of Service Discount. This discount will be applied on all charges paid for in full on the date that the services are provided and can help reduce the cost of your visit.
Payment for services is expected at the time of each visit. Please be sure to bring cash, check or a credit card with you.
