What can I do with my portal?
With our PatientPORTAL you can:
- Send and receive private NON-URGENT messages with your care team.
- Request a prescription refill.
- Request an appointment.
- View your medical records.
Additional features will be introduced throughout the year as we customize the Patient Portal to best serve the needs of our patients.
When will I receive a response to my message?
We will respond to your message within 2 business days. If you have an urgent question please CALL the office and DO NOT send a message using PatientPORTAL.
What parts of my medical record can I see and when can I see it?
Almost all diagnostic test results and clinical notes, with rare exceptions, will be immediately released to your patient portal without delay. Sometimes, this means that you may see your results BEFORE your Provider has seen them. Please understand that we look at every test result, and your provider will contact you for anything that needs follow up.
Notes not in your portal before April 5th will not be released retroactively. You can still contact your office to request a copy of any of your medical records
Should I wait to look at my results until after I’ve heard from my provider?
The 21st Century Cures act is a federal regulation that requires your medical record to be immediately available to you.
Using PatientPORTAL you can decide for yourself when it is the best time to see your results. You can view your results immediately after they are finalized, or you can wait until your provider’s office contacts you.
When your providers orders your tests, talk to them about what the best plan may be for you. Sometimes patients prefer to hear from their provider first regarding results that can potentially be life-altering, like the results of a biopsy.
How will I be notified that my results are available in the PatientPORTAL?
You will receive an email notification to view your results when your provider sends you a comment on your result.
Which results will I NOT be able to see?
- Certain adolescent tests that are protected by privacy laws.
- Test results that are sent to facilities that do not communicate with us electronically. These will come back to NCHC as a separate document.
Which notes can I see in PatientPORTAL?
All visits and documents from your medical record that occurred on or after April 5th, 2021 will be available online.
What about my family, caregiver or spouse?
Adult patients can choose to grant proxy access to their medical record to a family member or caregiver. This proxy access can later be removed by the adult patient at any time.
Remember that every part of your medical record is viewable by your proxy. Please keep this in mind when allowing another individual to be a proxy to your account.
You can always ask your provider to NOT send a note to your portal.
What about my children?
Parents may have access to their child’s medical record UNTIL THE AGE OF 12. When your child turns 12 your PatientPORTAL account will automatically be disconnected from their medical record.
We do not currently offer parental access to adolescent (Age 13-18) patient charts.
What can I do if I feel a provider has made an error in one of my visit notes OR if I feel there is an error with one of my results?
If you feel that your record needs to be amended or corrected, please send a message to your medical office team using PatientPORTAL or contact them directly by phone.
