Northern Counties Health Care, Inc. (NCHC) is a leader in integrating Behavioral Health (counseling) services within the Medical Home.
The Behavioral Health Specialist in each Health Center works closely with the medical providers and Chronic Care Coordinators in each Health Center to share information and coordinate care for patients in an efficient way.
Having caring and qualified Behavioral Health Specialists at our Health Centers means:
Convenience – Patients can receive mental health counseling, substance abuse screenings, and referrals to specialists all in one place.
Confidentiality – Having Behavioral Health services integrated within a primary care practice removes some of the stigma of receiving mental health services. No one knows why you’re there.
Accessibility – The large geographical span of NCHC health centers makes Behavioral Health accessible to many patients around the Northeast Kingdom and all NCHC practices are handicap accessible.
Help When You Need It – Behavioral Health Specialists are able to provide timely, sometimes emergency bridge services to patients in crisis who cannot find a therapist.
