Caledonia Home Health Care & Hospice’s (CHHC) Home Care program provides skilled, Medicare-certified, state-of-the-art home care services to promote health, minimize pain and suffering, allow you to remain safe in your home, and to be as independent and active as possible.
We work collaboratively with you, your family and your doctor to develop a plan of care that supports your safety and independence from the comfort of your own home.

Care is coordinated by a case manager who plans individualized home care according to each client’s health care needs and physician orders. Our team also includes a full range of home nursing and rehabilitation specialists. In addition, our nursing assistants provide help with bathing and other activities of daily living. Our social workers and mental health nurses provide counseling and referrals to community services.
Your home care begins with an assessment visit by a registered nurse or physical therapist, who will talk with you to determine your needs and a plan of care. Other members of the team also will visit, as needed. Your case manager will continue to be available to answer your questions and adjust your care. Visits will continue until your goals are met.

Recovery from Surgery or Short-Term Illness
If you need home care following surgery or a brief illness, Caledonia Home Health Care & Hospice can help. Whatever your rehabilitative needs, our qualified professionals are here to help you return to a full and rich life. Chronic Illness & Disease Management Services
Caledonia Home Health Care & Hospice offers skilled home care services to those learning to cope with the management of long-term disabilities or chronic illness. Our team of home care professionals works with you, your family, and your doctor in developing and providing a comprehensive plan of care to allow you to stay in your home setting. Whatever your needs, we have nurses available seven days a week, 24 hours a day, with day and evening scheduled visits and an on-call service at night.
Your care plan may include:

Physical, Occupational & Speech Therapy at Home
Caledonia Home Health Care & Hospice’s Home Rehabilitation Therapy Services are there for you when you need us. Our services allow you to receive rehabilitation care in the comfort of your own home after hospitalization, or as approved by your physician. We provide physical therapy, occupational therapy and speech therapy to people who have medical problems or other health related conditions that limit their ability to move and perform activities of daily living.

Elder Care Services
Caledonia Home Health Care & Hospice’s home elder care specialists are skilled in planning care to meet your individual needs. We understand your desire to be as independent as possible and to remain in your own home. CHHC offers a wide range of elder care services to support independence at home, with extra help as needed. You may want your family members or other caregivers to be involved in planning, communication and giving care. CHHC nurses, rehabilitation therapists, social workers and nursing assistants work with you and your family to provide the right care when it’s needed. When the unexpected happens, a CHHC nurse is always available to help decide how to manage the situation. We strive to protect your safety, health and comfort.
Home Safety and Fall Prevention
An expert physical therapist and/or occupational therapist will make a thorough assessment of your safety at home and develop an individual program to prevent falls and injury. Your care plan may include adding safety devices such as grab bars, learning safer ways to manage household activities, and doing home exercises to improve your strength and balance. A registered nurse may also be involved to help you safely manage your medications.
Dementia and Cognitive Impairment Care
CHHC’s elder care team is experienced in all aspects of caring for clients with mild to severe memory loss or cognitive impairment. Based on an individual assessment, the care team may include a mental health registered nurse, physical therapist, occupational therapist, speech therapist, medical social worker, and nursing assistants. A plan of care is developed to maximize safety and quality of life for both you and your family. The care plan addresses your medical, social, and emotional needs, as well as providing support for your family in managing the stress of caregiving. Community resources are included, whenever appropriate.
Many seniors take multiple medications every day. Understanding the purpose of these medications, and keeping track of when to take all the pills, can become a challenge. A CHHC elder care nurse can help sort out why, when and how to take all the medicines so that you can get the most benefit from them, and will know when to contact your health care provider about medication problems.
Home Telehealth/Tele-monitoring Services
Caledonia Home Health Care & Hospice’s Telehealth/Tele-monitoring Program monitors a client’s vital signs and symptoms from the client’s own home via a machine that transmits the information directly to us. Telehealth monitoring has been shown to improve the delivery of home care and to decrease unplanned emergency visits and hospitalizations.
Telehealth monitoring is helpful for home care clients with an unstable condition, those at risk for quickly developing complications or worsening of an illness, clients at risk of re-hospitalization or those with chronic diseases such as Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Asthma or High Blood Pressure/Hypertension. Telehealth monitoring may also be used to check the effectiveness of a change in medication or treatment.
How Does Telehealth Monitoring Work?
We will install and train the client to use the simple, non-invasive equipment in a client’s home. The easy to use equipment monitors vital signs which will be automatically sent via the phone line to our office for interpretation and follow-up.
At a pre-planned time every day, your monitor will prompt you to check your pulse, blood pressure, oxygen level, and weight. The telehealth monitor gives simple instructions to perform these measurements and there is plenty of time to complete each task, so there is no need to rush. After collecting the information, the monitor may ask some simple pre-programmed yes or no questions.
All the data is then transmitted via telephone line to our central station computer where your nurse checks the information daily. The telehealth nurse responds to changes in vital signs and symptoms by contacting you to discuss the change. The conversation may result in a repeat test, education about how you can control your symptoms, a home visit and/or treatment changes to prevent worsening of your condition. As needed, the telehealth nurse consults your nurse and/or physician to discuss the best way to help you be healthy, safe and comfortable.
