Payment Information
Northern Counties Health Care, Inc. accepts all patients regardless of their insurance or financial status. We accept most private insurances, Medicaid, and Medicare. Our federally funded health care program allows us to offer a wide range of services to patients through a sliding fee scale program.
Sliding Fee Discount Program
- Northern Counties offers reduced fees to patients who meet income guidelines as set forth each year by the Federal Government.
- Eligibility is based on both income and family size. The program is offered to make health care more affordable for those who need assistance.
- To enroll patients:
- Provide proof of residency (Vermont Drivers License, Voter Registration Card, or Vermont State Income Tax Return); and
- Complete an application form with documentation of yearly income (generally your most recent Federal Tax Return).

- Depending on which sliding scale level qualified for, fees are reduced to a lower specific percentage of charges or to a fixed charge per service.
- Enrollment or re-enrollment is required each year.
- Members of our health center staff can answer questions and help with the application process.
