Medical Home
Northern Counties Health Care, Inc. (NCHC) has six health care centers, which are all Federally Qualified Health Centers and have received recognition as a Patient Centered Medical Homes. These health centers located in Concord, Danville, Hardwick, Island Pond, and St. Johnsbury received this recognition from the National Committee for Quality Assurance. NCHC was among the first in Vermont to receive this recognition.
The Medical Home takes a team approach, with the patient at the center of the team, to provide quality patient care to maintain a person’s health. This approach involves coordination of care for our patients beyond primary care and reduces the risk that a medical problem will worsen and require a hospital visit. Involvement of the Community Health Team (CHT) within the region looks to provide resources that can enhance the access and delivery of health care for our patients.
