COUNTIES HEALTH CARE, Inc.
Full Time RN Staff Nurse
Nursing duties include patient triage, taking
patient history, and vital signs; nursing assessments,
administering injections, and performing some laboratory
testing.
Applicants must have the ability to work well with patients and
other staff, a high energy level, good communication skills,
computer literacy and a willingness to learn.
VT licensure as a Registered Nurse is required.
A minimum of 1 year of primary care experience is preferred
Behavioral Health Specialists
Provide triage, assessment, crisis management and short-term
counseling interventions in an integrated primary care/
behavioral health model. LCSW, LCMHC or Licensed Masters
Psychologist required. Prior counseling experience, knowledge of
DSM-IV-TR and knowledge of primary care behavioral health needs
preferred.
At Northern Counties Dental Center: PART TIME DENTAL HYGIENIST
Requires VT Licensure. At least 1 year of experience is
preferred.
PART TIME TO FULL TIME DENTIST~
FOR PATIENTS OF ALL AGES AT EITHER OF OUR FQHC SITES: Hardwick /
Island Pond
Requires DDS or DMD from accredited school and VT Licensure
A minimum of 1 year of practice or dental residency preferred.

Physical Therapist
Provide Physical Therapy services to homebound clients
throughout our service area. Must be able to work with a team,
as well as independently.
Applicants must have a Vermont Driver’s license, reliable
transportation and Vermont licensure as a Physical Therapist with at
least one year of experience in a health care setting.
Community Health Staff Nurse, RN
PT position with weekend rotation providing home care services
in Caledonia and southern Essex counties.
Requirements include having a valid driver’s license, a reliable
vehicle, and licensure in the State of Vermont as an RN.
Home Health and supervisory experience preferred
