Hospice and Palliative Care Services
Our Hospice program offers clients with the highest quality of life possible when they have a life-limiting illness. We offer a comprehensive and coordinated approach to care by utilizing the expertise and services of our Medical Director, Hospice Chaplain, Social Services, Nursing Care, Volunteer services, the client’s own physician and paraprofessional services.
Who is eligible for Hospice services?
Anyone is eligible for Hospice care if they have a life threatening/life limiting illness when the client, family, caregiver and physician agree to this level of care. Hospice care can be provided in a client’s own home, a family members home, a paid caregivers home or a nursing home.
Services offered:
- Hospice services focus on management of symptoms that are intended to allow the client to have quality time with their families and friends.
- All care is individualized with the client , caregiver, family and physician input.
- Music Therapy is available to clients. Our music is provided by a group of volunteers who will visit the home and sing songs that are comforting to our client/family.
- Pet Therapy is available for those individuals who would like a visit from one of our volunteer’s therapy dogs.
- Volunteer services are available to give respite to caregivers, to read to the client, play games with the client , run errands or just quietly sit nearby.
- Sweet Relief offers homemade goodies for clients and their families.
- We offer massage therapy and Reiki therapy that can provide physical comfort and relief of painful symptoms.
- We provide state of the art pain and symptom management.
- Spiritual care is available for clients and families.
- Bereavement services are offered in a variety of ways. Families are encouraged to attend our bi-monthly support groups or could have an individual bereavement volunteer visit in the home.
- Professional skilled nursing services monitor symptoms and make necessary changes in the care delivered to assure comfort and dignity.
- Homemaking services may be provided.
