Home Health Services
Caledonia Home Health Care provides intermittent, medically necessary nursing and rehabilitative therapies to clients in their own homes. Referrals for this service come from a variety of sources; the doctor’s office, hospitals, family members, friends and clients themselves.
- Home Care Services
Our nurses provide a wide range of services including wound care, IV therapy, disease specific teaching, medication management, physical assessments, pain management and blood drawing. All care is developed in conjunction with a plan of care developed by the nurse, the client and the attending physician.
- Rehabilitation Services : Physical Therapy, Occupational Therapy and Speech Therapy
Our Physical Therapy staff works with individual clients to gain optimal mobility, range of motion of all extremities, and independence with walking. Occupational therapy works closely with a client to increase optimal independence with such functions as dressing, meal preparation, bathing and suggesting modifications to the home for safety. Speech therapy works with clients on issues related to speech and swallowing. They will develop an individualized care plan that addresses a client’s needs for exercise and rehabilitation with the client’s physician.
- Medical Social Services
Our Medical Social Services staff works with clients on behavioral issues as well as coordinating necessary community services. They will assist the client in developing the necessary coping skills to deal with illness.
- Licensed Nursing Assistants
Our Licensed Nursing Assistants called Home Care Aides will assist clients with bathing, exercises, uncomplicated medical therapies, some light housekeeping and meal preparation. They work under the direction of the nursing staff and the rehabilitation staff by following a plan of care designed to meet each client’s needs.
- Tele-monitoring Services
For clients at risk of re-hospitalization or those with acute exacerbations of an illness may be offered our tele-monitoring services. We will install the simple, non-invasive equipment in a client’s home that can monitor vital signs, weight, fluid levels in the bottom, blood sugar and protimes. The information gathered will be automatically sent via the phone line to our office for interpretation and follow-up.
- High Technology Nursing
We have a highly trained staff that can assist families and clients in receiving these services in the home. This might include IV therapy, maintenance of central lines, PICC lines, babies or children with hi-tech needs, and tube feedings.
- Intake Coordination
Our intake department will gather the necessary information on a potential client. The professional staff then will make a visit to the client to assess whether Caledonia Home Health Care services are appropriate.
Long Term Care
Our long term care program is designed for frail elders or those with debilitating conditions that require additional services so they may remain in their own homes. Our staff provides assistance with activities of daily living such as housekeeping, meal preparation, companionship and respite services.
Services:
- Homemaker assistance with housekeeping, laundry, meal preparation, shopping and errands.
- The Choices for Care program works under a plan of care developed in conjunction with state regulations that offer an alternative to nursing home care. Under this service, clients can receive case management services , homemaking, personal care assistance, meals, companionship and respite care.
Maternal Child Health
Our Maternal Child Health Program is designed to provide teaching and assessment to pregnant women, new mothers and babies.
Services:
- Care is provided by a registered nurse. Visits will be made, as necessary, to provide teaching, monitor development, provide coordination of services and assist pregnant women, new mothers and babies in developing the necessary skills for better beginnings.
- Family Support Worker services are provided to children and families to assist in the development of positive interaction between family members as well as assisting in growth development.
