As a non-profit division of Northern Counties Health Care, Inc., Caledonia Home Health and Hospice’s mission is to enhance the quality of life of individual’s and families by providing compassionate, skilled, accessible, and affordable patient-centered health services for members of Vermont’s Northeast Kingdom.
We envision a community who is continually supported by a skilled and caring team of healthcare providers that offer clients the highest quality of life and independence possible when faced with illness. As the primary home health provider for our community, our focus is on comfort and care provided in the home and appreciating the value and contribution of each family member.
The CHHC Annual Fund provides the unrestricted resources necessary to support elements of the following critical programs for CHHC that are not covered by state and federal funding:
Our Long-term Care Program provides case management, homemaker, and personal care services to chronically ill individuals to allow them to remain at home.
Our Home Care Program offers short-term assistance to individuals and families with acute or chronic illness, following a change in their needs, i.e., a recent hospitalization, new diagnosis or a change in medication. Our team includes a full range of home nursing and rehabilitation therapists who perform skilled interventions and provide specialized education. In addition, our nursing assistants provide help with activities of daily living and our social workers provide counseling and referrals to community services.
Our Hospice Program helps individuals with terminal illness by offering a comprehensive and coordinated approach to care by utilizing the expertise and services of our Medical Director, Hospice Chaplain, Social Services, Nursing Care, Volunteer Services, and the individual’s own physician and paraprofessional services to provide our patients with the physical, emotional, spiritual and psychosocial support they need to focus on living fully and comfortably.
Our Maternal & Child Health Program is designed to provide medical treatment and preventative education to pregnant women, new mothers and children, ranging from IV therapy, blood pressure monitoring and wound care to support with breastfeeding and child development.
Community Outreach and Professional Development – Staff training, computers and network support, community outreach and education programs and workshops, transportation costs, and certifications.

Giving appreciated securities is a perfect way to show support for CHHC. With certain limitations, you can deduct the full fair market value of long-term appreciated securities – that is, securities you have owned for more than one year and that have increased in value. Thus you can give away appreciated property and usually avoid the tax on the gain. Always consult your financial planner for appropriate guidance and details.
