Performance Improvement / QI Manager
Qualified candidate will oversee Northern Counties’ clinical quality performance improvement program. Skills required include: demonstrated experience with process improvement initiatives and methodologies, demonstrated leadership, primary care background, strong EMR skills. Bachelor’s Degree in management, public health, nursing, business administration, organizational leadership or related field. Minimum five years of progressively responsible experience in health care.
Per Diem RNs
At our Community Health Centers in Danville, Concord, Hardwick, Island Pond, and St. Johnsbury as well as at Caledonia Home Health & Hospice
Behavioral Health Specialist – St. Johnsbury
Provide triage, assessment, crisis management and short-term counseling interventions in an integrated primary care/ behavioral health model. LCSW, LCMHC or Licensed Masters Psychologist required. Prior counseling experience, knowledge of DSM-IV-TR and knowledge of primary care behavioral health needs preferred.
This is a part time (24 hours per week), benefitted position.
Per Diem Medical Receptionist - Float
We are looking for candidates who have excellent communication, computer, and customer service skills;
Must be energetic, organized, friendly, flexible, and sensitive to confidentiality, and possess the ability
to multi-task. Must be comfortable working as a member of a team as well as independently.
Physician
We seek an energetic, team-oriented physician, preferably with Med/ Peds, Internal Medicine, or Family Practice experience to join the clinical staff within the network of our community based health centers. Northern Counties currently has six rural health centers serving the residents of the Northeast Kingdom of Vermont. We are uniquely privileged to offer sliding scale fees for medical, lab, pharmacy and dental services, thus providing affordable care to a vibrant rural community. The area offers outstanding year-round recreational opportunities, affordability and good schools, and is located approximately 2 hours from Burlington and Montreal and 3 hours from Boston. Candidates must be BE/BC and will require licensing in the State of Vermont. Competitive salary and benefits along with Federal and State loan repayment eligibility. EOE.

Part Time Community Health Staff Nurse, RN
Per Diem position at Caledonia Home Health and Hospice providing home care services in Caledonia and southern Essex counties. Requirements include having a valid driver’s license, a reliable vehicle, CPR Certification and licensure in the State of Vermont as an RN. Home Health and supervisory experience preferred.
